Our public website does not collect protected health information (PHI). The contact form on this site collects only the information you choose to share: your name, email address, optional phone number, state, who therapy is for, preferred contact method, and best contact time. We use this information only to respond to your consultation request. The form does not ask for medical details.

Protected health information
Once you become a patient, additional health information is collected as part of your clinical care. Testimonials
Testimonials are published only with written permission. Some testimonials include health details that the person chose to share and specifically approved for public display. We do not collect those health details through the website consultation form.
